When dying red blood cells outpace the bone marrow’s production of new ones, hemolytic anemia occurs. Hemolytic anemia can be extrinsic or intrinsic. World Osteoporosis Day 2017 aims to spread awareness about the disease, so that This year’s objectives of the World Osteoporosis 2017. Anemia by Medical dictionaryanemia [ah- ne´me- ah] a condition in which there is reduced delivery of oxygen to the tissues; it is not actually a disease but rather a symptom of any of numerous different disorders and other conditions. The World Health Organization has defined anemia as a hemoglobin concentration below 7. L (1. 2 g/d. L) in women and below 8. L (1. 3 g/d. L) in men. Peripheral blood smears from a patient with megaloblastic anemia (left) and from a normal subject (right), both at the same magnification. The smear from the patient shows variation in the size and shape of erythrocytes and the presence of macro- ovalocytes. From Goldman and Bennett, 2. Some types of anemia are named for the factors causing them: poor diet (nutritional anemia), excessive blood loss (hemorrhagic anemia), congenital defects of hemoglobin (hypochromic anemia), exposure to industrial poisons, diseases of the bone marrow (aplastic anemia and hypoplastic anemia), or any other disorder that upsets the balance between blood loss through bleeding or destruction of blood cells and production of blood cells. Anemias can also be classified according to the morphologic characteristics of the erythrocytes, such as size (microcytic, macrocytic, and normocytic anemias) and color or hemoglobin concentration (hypochromic anemia). A type called hypochromic microcytic anemia is characterized by very small erythrocytes that have low hemoglobin concentration and hence poor coloration. Data used to identify anemia types include the erythrocyte indices: (1) mean corpuscular volume (MCV), the average erythrocyte volume; (2) mean corpuscular hemoglobin (MCH), the average amount of hemoglobin per erythrocyte; and (3) mean corpuscular hemoglobin concentration (MCHC), the average concentration of hemoglobin in erythrocytes. Symptoms. Mild degrees of anemia often cause only slight and vague symptoms, perhaps nothing more than easy fatigue or a lack of energy. As the condition progresses, more severe symptoms may be experienced, such as shortness of breath, pounding of the heart, and a rapid pulse; these are caused by the inability of anemic blood to supply the body tissues with enough oxygen. Pallor, particularly in the palms of the hands, the fingernails, and the conjunctiva (the lining of the eyelids), may also indicate anemia. In very advanced cases, swelling of the ankles and other evidence of heart failure may appear. Common Causes of Anemia. Loss of Blood (Hemorrhagic Anemia): If there is massive bleeding from a wound or other lesion, the body may lose enough blood to cause severe and acute anemia, which is often accompanied by shock. Immediate transfusions are generally required to replace the lost blood. Chronic blood loss, such as excessive menstrual flow, or slow loss of blood from an ulcer or cancer of the gastrointestinal tract, may also lead to anemia. These anemias disappear when the cause has been found and corrected. To help the blood replenish itself, the health care provider may prescribe medicines containing iron, which is necessary to build hemoglobin, and foods with high iron content, such as kidney and navy beans, liver, spinach, and whole wheat bread. Dietary Deficiencies and Abnormalities of Red Blood Cell Production (Nutritional Anemia, Aplastic Anemia, and Hypoplastic Anemia): Anemia may develop if the diet does not provide enough iron, protein, vitamin B1. The combination of poor diet and chronic loss of blood makes for particular susceptibility to severe anemia. Anemias associated with folic acid deficiency are very common. Excessive Destruction of Red Blood Cells (hemolytic anemia): Anemia may also develop related to hemolysis due to trauma, chemical agents or medications (toxic hemolytic anemia), infectious disease, isoimmune hemolytic reactions, autoimmune disorders, and the paroxysmal hemoglobinurias. Patient Care. Assessment of patients with some form of anemia will depend to some extent on the specific type of blood dyscrasia presented. In general, these patients do share some common problems requiring special assessment skills and interventions. Anemia can affect many different body systems (see table). Although pallor of the skin is a sign of anemia, it is not the most reliable sign; many other factors can affect complexion and skin color. Jaundice of the skin and sclera can occur as a result of hemolysis and the release of bilirubin into the blood stream, where it eventually finds its way into the skin and mucous membranes. See also jaundice.) Bleeding under the skin and bruises in response to the slightest trauma often are present in anemic and leukemic patients. A bluish tint to the skin (cyanosis) can indicate hypoxia due to inadequate numbers of oxygen- bearing erythrocytes. Activity intolerance is a common problem for patients with anemia. Intravenous (IV) infusion: Use administration set provided by the manufacturer. Administer using a large needle or catheter of at least 2. For hypovolemic shock: The initial dose should be administered as rapidly as tolerated. After the initial dose is administered, the rate of administration should be adapted to the response of the patient. As blood volume approaches normal, the infusion rate should be slow enough (1 m. L/minute) to prevent too rapid expansion of plasma volume. For hypoproteinemic patients: Administration rates for the 2. Maximum rates of 2 m. L/minute and 3 m. L/minute are recommended. Too rapid infusion may cause circulatory compromise, including pulmonary edema. MICRho. GAM/Rho. GAM (rho(D) immune globulin (human)) dose, indications, adverse effects, interactions.. PDR. net. CLASSESOther Disease Specific Immunoglobulins. BOXED WARNINGAnemia, hemolysis, hemolytic anemia, renal impairment, requires a specialized care setting Win. Rho SDF and Rhophylac are approved for intravenous use in patients with idiopathic thrombocytopenic purpura (ITP). Increased destruction of Rh. D]- positive red cells occurs after Rh. D] immune globulin administration, and this will produce decreased serum hemoglobin concentrations and associated clinical symptoms. Because of this, use with caution in patients with preexisting anemia. Patients with ITP who have a hemoglobin < 1. Win. Rho SDF in order to decrease the risk of worsening the severity of the anemia. Alternative treatments should be used in patients with ITP who have a hemoglobin level < 8 g/dl because the risk of the severity of anemia is increased. The safety of Rhophylac in the treatment of ITP has not been established in patients with pre- existing anemia; weigh the benefits of Rhophylac versus the potential risk of increasing the anemia severity. While most of the red cell destruction will occur in the spleen, there have been rare reports of acute hemoglobinuria consistent with intravascular hemolysis that have occurred during Rh. D] immune globulin administration to patients with ITP. Win. Rho SDF is contraindicated for use in patients with pre- existing hemolysis, patients at high risk for hemolysis, and in patients with autoimmune hemolytic anemia. Rh. 0[D]- positive patients with ITP should be monitored for signs and/or symptoms of intravascular hemolysis (IVH), clinically compromising hemolytic anemia, and renal impairment during treatment. IVH leading to death has been reported in patients treated for ITP with Rh. D] immune globulin. IVH can lead to clinically compromising anemia and multi- system organ failure including acute respiratory distress syndrome (ARDS). Serious complications, including severe anemia, acute renal impairment, renal failure, and disseminated intravascular coagulation (DIC) have also been reported. Win. Rho SDF and Rhophylac for ITP requires a specialized care setting; closely monitor patients in a health care setting for at least 8 hours after administration. Perform a dipstick urinalysis at baseline and at 2 hours, 4 hours, and 8 hours after administration. Monitor signs and symptoms of IVH including back pain, chills, fever, and hematuria. Absence of these signs and/or symptoms of IVH within 8 hours does not indicate that IVH cannot occur subsequently. If signs and/or symptoms of IVH are present or suspected after Rh. D] immune globulin administration, post- treatment laboratory tests should be performed including plasma hemoglobin, microscopic and dipstick urinalysis, haptoglobin, LDH, and plasma bilirubin (direct and indirect). Periodic monitoring of renal function and urine output is particularly important in patients judged to be at increased risk of developing acute renal failure. Assess renal function in all patients before initiation of Rh. D] immune globulin and at appropriate intervals thereafter for at- risk patients. Infusion of intravenous immune globulin at a minimal practical concentration and infusion rate is recommended for patients with renal insufficiency or for patients who are predisposed to acute renal failure. Receipt of intravenous immune globulin has been reported to produce renal dysfunction in these patients. Most reports of renal dysfunction have involved products that contain sucrose as a stabilizer. If renal dysfunction occurs, use clinical judgment to either decrease the infusion rate or stop the infusion. DESCRIPTIONParenteral Ig. The Fridge Brilliance trope as used in popular culture. You watch a movie and something about it just seems off, you don't like it. One night, as you get up. IGN counts down the best horror movies of all time! Well good news, there's an incredible array of board games available now that are miles away from repeated dice rolls of old school Monopoly or the well named Frustration. From the fiendish atmosphere and replay value of Betrayal at House on the Hill, to the co- operative feel of Pandemic Legacy, today's board games make a pleasant alternative to video games and there's nothing like clustering around the table to share in an adventure. There's slews of brilliant options out there but how are you to know which is the best board game? To help you navigate this rollercoaster of quality, here's a list of the eight best board games around. We've leaned toward titles you can learn and play in an evening, and which scale to suit a range of player numbers. As a rough guide, they're presented in order of accessibility, with the easier to understand games first. Codenames. The party game with actual strategy. Players: 2- 8 | Time to set up: Two minutes | Time to play: 1. Complexity: Easy | Age: 1. A word game that anyone can play. Scales well with any number of players. Has moments of silent downtime. Occasional arbitrary game end. Most party games rely on silliness or trivia to function. Codenames is a clever design that also throws a modicum of strategy and skill into the mix. One player invents single- word clues that guide their team- mates toward particular word cards laid out in a grid. The clue can be anything: it might rhyme with the target, or make a compound word, or be a synonym. The team doesn't know and the clue giver isn't allowed to say. Cue the same levels of discussion, amusement and frustration as your average party game with a ton more mental stimulation. It's harder to come up with clues than it sounds, which can lead to moments of quiet as they desperately think of links. Not the best look for the middle of your drunken knees- up. But it also makes things far more exciting, because guessing wrong can sometimes score points for the other team, or even result in an instant loss. If that's too harsh, there's a co- operative variant in the box. There's also a separate Pictures version that uses images instead of words on the cards. Best for: .. after dinner or party play. King of Tokyo. Smash up a city, and some friends at the same time. Players: 2- 6 | Time to set up: Five minutes | Time to play: 3. Complexity: Easy | Age: 8+Exciting dice battle with social elements. Conveys a fun theme with little effort. Little strategy. King of Tokyo is a game about being Godzilla, or one of several other silly super- monsters, crashing through Tokyo. But in a genius piece of abstraction, there is no city map. Instead, you compete with your fellow monsters to be the one doing the smashing each turn. This gives you points, as does buying cards representing mass destruction. Other cards enhance your monster with powers like extra heads, poison spit or a spiked tail. These you can use to fight the other monsters, and being the last one standing is just as realistic a route to victory as crushing the most city blocks. All this gets resolved via a Yahtzee style mechanic that you can explain to anyone in seconds. Throwing fistfuls of custom dice around is brilliant fun. And there's a social element too, as players conspire to topple the monster in the city, while each hoping to be the one to take its place. While enjoyable as a mindless, drunken romp it's also open to some strategy in the choice of cards you buy. If you're willing to trade a few more rules for a bit more tactics, consider the King of New York version instead. Best For: .. drunken knees- ups. Ticket to Ride. Cut- throat family fun. Players: 2- 5 | Time to set up: Five minutes | Time to play: 3. Complexity: Easy | Age: 8+Easy to pick up and play, even for kids. Delightfully vicious. Can get repetitive. Everyone knows set collection playing card game Rummy; everyone also knows it's samey and luck driven. Ticket to Ride takes the concept and adds a board representing a map of America. Players cash in sets of matching train cards to claim routes, vying to connect important cities with long chains to maximise their score. It's a tense race to collect the colours you need before one of your opponents, but only one player can own each route, so half the fun is swiping critical junctions from under everyone else's nose. It's a simple, family- friendly game that's unlikely to make for family harmony. Once you've learned the most important routes, that simplicity starts to feel a bit repetitive. To compensate, the game has spawned a slew of variants and expansions of varying quality. The Europe and Nordic Countries editions trade complexity for more strategy and more viciousness, respectively. The rest are best ignored. If you're unsure, you can try the original and most of the variants out via an excellent mobile version on the app store and Google play. Best For: .. playing with family and friends. The Truth About Attention Deficit Hyperactivity Disorder. With her long dark hair flying, Saorla Meenagh, 1. Irish step- dancing style. Saorla, whose blue eyes, white skin, and sprinkle of freckles helped win her a modeling contract with a New York agency—on hold until her braces come off —also plays softball and Gaelic football, a soccer- like game her father, Seamus, a contractor, enjoyed when he was growing up in County Tyrone, Northern Ireland. When asked her position on the team, Saorla wiggles in her seat at the family’s dining room table. I don’t know—random positions,” she says, as her mother, Kerri, a pediatric nurse practitioner quietly pushes her daughter’s bare foot off the table where it has wandered for what may be the 1. During that time, Saorla’s actions are as scattered as pool balls in a break. She sits on her knees, then jiggles, then rocks, then circles photos in a stack of food magazines with a pen, turning off the TV, turning up the TV, turning down the TV, and making part of a bracelet on a Rainbow Loom before she finally loses interest. She rarely finishes a task and being with her can be as disconcerting as watching television with someone pressing the channel changer every 1. Along with a gift for math and a love for Gaelic sports, Saorla has inherited something else from her father: attention deficit hyperactivity disorder, ADHD. People just meeting her find her adorably sweet, if quirky. But if she’s not on her medication—which she isn’t at the moment because we met at her family’s home in a Philadelphia suburb during the summer—she is an exhausting ball of boundless energy. While she’s not at school, her mother gives her a medication holiday in order to boost her appetite, help her gain weight and grow, all things Vyvance interferes with.) “When we had her tested they said that in 2. Meenagh, gently removing that errant foot from the table yet again. But Saorla’s perpetual motion isn’t what concerns Meenagh the most. Being fidgety and easily distracted are two of the most common and recognized symptoms of ADHD, often leading to poor performance in school, the most recognized fallout of the condition. But the 5% to 1. 1% of American children 4 to 1. Overall, boys (1. ADHD diagnosis. It’s the fear for her child’s future that makes Meenagh bristle when she hears someone—family, friends, strangers, even scientists—say that ADHD doesn’t exist, that the symptoms are caused by poor parenting, food additives, or 2. A lot of people don’t believe in it,” she says. I think that’s because they’ve never experienced it firsthand.”To those naysayers, C. Keith Connors, professor emeritus of medical psychology at Duke University and creator of the Connors Rating Scales for diagnosing ADHD, says he always offers the same challenge: “Take one of these kids on a car trip for a day and see how you feel about it then.”It’s easy to see why some dismiss ADHD as simply an artifact of modern life. After all, we’re all on the information Autobahn. Most Americans are exposed to an average of 1. Mark Twain’s The Adventures of Huckleberry Finn—very little of which we’re able to absorb, according to a 2. America’s information consumption from the University of California, San Diego. We’re also asked from a very young age to be still, nearly motionless, except for the tapping of our fingers on the computer keys. Ours is not a society tolerant of perpetual motion or daydreaming. At some point, we’re all like the dog in the Pixar movie, Up, doing one thing and then responding to the real- life equivalent of “Squirrel!”But ADHD isn’t a disorder of the modern age. It may have been first described in the medical literature in 1. Scottish physician Sir Arthur Crichton, who observed patients so unable to focus that “the barking of dogs, an ill- tuned organ, or the scolding of women, are sufficient to distract patients of this description to such a degree, as almost approaches to the nature of delirium.” Those patients, he noted, referred to their own symptoms, including anger “bordering on insanity,” as “the fidgets.”The argument doesn’t hold up either when ADHD’s worldwide prevalence is factored in. It’s found in developing countries, where the information speed limit is slower and the requirement to be sedentary 2. Yet there’s an undeniable backlash against the diagnosis. Earlier this year, Chicago neurologist and ADHD specialist Dr. Richard Saul published a provocatively titled book, ADHD Does Not Exist, in which he proposes that ADHD is not an entity in itself but a cluster of symptoms that stem from more than 2. Many of those, along with dysfunctional behavior conditions such as oppositional defiant disorder, characterized by aggression, tantrums and a propensity to argue with adults, and learning disabilities such as dyslexia, are often diagnosed as co- illnesses with ADHD. Saul criticizes the catchall- nature of an ADHD diagnosis, which he believes corrals a lot of real conditions into one. ADHD Across the Lifespan. Topics included on this page: • Infants, Toddlers and Preschoolers• Middle Childhood• Adolescence• Adulthood. More often than not, symptoms of ADHD will persist into adulthood. Obviously, the manifestation of ADHD symptoms differs across the lifespan. ADHD affects young children far differently than it does adults and the impact of symptoms, resulting impairment and methods of treatment vary by age and level of development. This section looks at ADHD at different stages of development: infants, toddlers and preschoolers; middle childhood; adolescence; and adulthood. Infants. Toddlers and Preschoolers. Although about ten percent of infants appear to have difficult temperaments characterized by irritability, hyperarousal, overactivity and inability to be easily calmed, clinicians do not diagnose ADHD in infancy. However, the overactive, temperamental infant is at greater risk to become the hyperactive, difficult preschooler. It is customary for preschool age children to be overactive and impulsive from time to time. Their attention is captured by things that interest them, but usually for short periods of time. They shift quickly from one activity to another. Have ADHD and ADD symptoms turned your child into an addict? Learn about this all-natural, drug-free ADHD solution whose only side effects are positive effects. We expect preschoolers to be somewhat demanding, impulsive or self- centered and generally we don’t get too upset when they get frustrated and have occasional temper tantrums or crying spells. Hopefully, we anticipate their frustration, plan for their short attention span and vary their activities enough to sustain their interests. At what point does activity exceed the bounds of normalcy and become hyperactivity? When is inattentiveness considered attention deficit? At what age should we expect immaturity to end and impulsivity to disappear? ADHD, identified as attention deficit hyperactivity disorder, is the name bestowed on a handful of behaviors that can have an effect on both children and adults, but.
Unfortunately there are no objective answers to these questions. Healthcare professionals must be careful to differentiate normal preschooler hyperactivity and impulsiveness from the “over the top” behavior characteristic of preschoolers with ADHD. For example, some young children have language delays. They may exhibit hyperactivity and disruptive behavior out of frustration arising from their inability to communicate. Some young children may simply not be mature enough to handle the demands of preschool and the routines of their busy parents. They act out. Dr. Susan Campbell studied three- year- olds with a pattern of hyperactive, impulsive and related disruptive behavior. Nearly one half continue to experience behavioral problems by school age, and nearly one- third receive a diagnosis of ADHD. Parents of ADHD preschoolers often describe themselves as exhausted by the child- rearing process. The typical methods of discipline such as time- out, positive reinforcement and punishment don’t work well. They may try to discipline their youngsters and teach appropriate behavior, but the child seems unable to learn. Parents are often left without an effective means of control. Teachers of ADHD preschoolers end up equally frustrated. Disruptive behavior, especially aggression toward other children, becomes a chief concern for the teacher. Parents of the more seriously involved kids are frequently asked to withdraw their child from the preschool only to end up in a desperate search for another school that will be able to handle their child’s problems. Such places are rare. Fortunately, the majority of the preschool- age children described by their parents as inattentive and overactive will show improvement in these areas as they mature. However, for those children whose hyperactivity and conduct problems persist for at least one year, there is a much stronger likelihood that they will have continuing difficulty in these areas and may more likely receive a diagnosis of ADHD or oppositional defiant disorder (ODD) in the future. Middle. Childhood. Many children with ADHD will be identified by the time they are nine or ten years of age. If the child is primarily inattentive, but not necessarily hyperactive, he or she will leave a trail of unfinished tasks: uncovered toothpaste in the bathroom, clothes scattered about the floor of the bedroom, bed unmade, toys and books left wherever they were last used, games started and unfinished, tomorrow’s math homework paper. If hyperactivity and impulsivity are also present, the picture at home becomes even more chaotic: toys scattered and broken, walls marked up, frequent family arguments over listening, meals disrupted by fighting, shopping trips marred by relentless demands, sibling conflicts and frayed nerves. Patterns of academic frustration and failure, social rejection and criticism from parents and teachers build in elementary school to the point where other disorders associated with ADHD begin to appear. Approximately 6. 0 percent of these children will develop oppositional disorder, characterized by defiant and non- compliant behavior. Frustrated by their lack of success, these children may become irritable and sullen. Aladdin | Disney Wiki | FANDOM powered by Wikia. Aladdin is the protagonist of the 1. Disneyanimated feature film of the same name. Also known as the "diamond in the rough", Aladdin was born a street rat and lived most of his youth in poverty, though he maintained a warm heart and selfless nature in spite of his struggles. He would become a prince after marrying Princess Jasmine, with whom he fell deeply in love with. Aladdin is based on the eponymous character of the Aladdin folk tales featured in the Middle Eastern and South Asian One Thousand and One Nights stories. Background. Aladdin was born to Cassim and his wife. Even then, Aladdin's background was one of struggle, prompting his father to leave Agrabah in hopes of finding a way to better provide for his family. After his leave, Aladdin's mother fell ill and she passed away, and Aladdin was left to fend for himself. By the time Cassim returned, he couldn't find his family, believing them to be dead, resulting in him leaving Agrabah out of despair, where he'd eventually become a leader of the infamous Forty Thieves, though this would remain unknown to Aladdin for an extended number of years. Due to being orphaned so early, Aladdin never received a formal education, nor had a positive role model to guide him through life. With no form of income, he was forced to learn to survive on the streets of Agrabah. Lacking other means, he steals only to survive, but his good heart often moves him to donate his stolen goods to those who are as poor, if not poorer than him, making him a Robin Hood- esque figure. When Aladdin was seven, he had his first encounter with Razoul, the new captain of the Sultan's guard, after stealing an apple from a fruit stand. Initially, the boy managed to outmaneuver the guards, but was eventually apprehended and sentenced to detention within the palace dungeon; however, he managed to escape by picking the locks to his chains. While in his mid- teen years, he stole a vest, a pair of pants, and a fez from a clothes line, which would form his signature outfit. When he was sixteen, Aladdin fell in with a group of circus performers: Aziz, Fatima, Minos, and their pet, Abu the Monkey. Aladdin and Abu would form a brotherly relationship throughout this endeavor, and would eventually leave the circus, but remain together as life partners. Development. In the earliest stages of the story, Aladdin was portrayed as a much younger character—a boy, as opposed to a young man. The idea was brought upon by animator Glen Keane, who believed Aladdin, the lead of an underdog story should have a more meager, vulnerable appearance to match his role in the film's narrative.
